Requirements

  • Certified and licensed Nurse Practitioner in Washington; ANCC (or equivalent) in Adult/Family/Geriatric/Acute
  • Eligible for Medicare participation
  • 2+ years NP experience with complex adult populations; able to practice independently
  • Valid Washington driver’s license; comfortable traveling locally (throughout the state of Washington, including 30–40-mile radius coverage)
  • Tech-savvy: Google Suite, multiple EMRs, Slack, AI transcription/tools
  • Thrives in a dynamic, evolving startup environment
  • English required; Spanish preferred

Responsibilities

  • Direct patient care: Conduct initial and follow-up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers
  • Tech → bedside integration: Train and coach staff at the point of care, standardize use of Circadia into clinical workflows, and run live demos during real patient encounters
  • Data into action: Interpret trend-based insights, prioritize high-risk patients, and drive timely interventions to prevent decline
  • Close the loop: Document outcomes, refine protocols, and lead brief huddles to sustain adoption and measurable impact
  • High-volume rounding: Efficiently complete visits across designated SNFs (typ. 25+/day where appropriate), focusing on new admits, facility-flagged patients, and Circadia alerts
  • Quality & product feedback: Surface field insights to improve workflows, summaries, and flagging logic; participate in root-cause reviews
  • Mentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams
  • Regional flexibility: Cover nearby sites as needed
