Candidates must possess a Bachelor of Science in Nursing (BSN), reside in a compact NLC state, and hold an active Compact RN license in good standing. A willingness to become licensed in multiple states and current CCM Certification or eligibility for CCM by August are required. The role necessitates over 5 years of nursing experience, with at least 2 years specifically in a Newborn/Pediatric setting. Applicants must be available to work Monday-Friday, 9 am-6 pm PST, be comfortable discussing multiple medical conditions, and strictly adhere to security and HIPAA regulations. Strong computer system proficiency, the ability to collaborate with diverse teams, and support for members with cultural and socioeconomic issues are also essential.
The Newborn/Pediatric Nurse Care Manager will deliver coordinated, patient-centered virtual care management via telephone and video to improve member health outcomes. Responsibilities include creating care plans with members and the care team, helping members achieve health goals, and guiding them through complex medical conditions, treatment pathways, benefits, and the healthcare system. The role involves partnering with local providers for coordinated care, providing compassionate, longitudinal follow-up, and assisting during acute healthcare episodes like hospitalizations. Additionally, the Nurse Care Manager will coordinate necessary resources to holistically address member problems, whether clinical or social.
