Requirements

  • Functions as Registered Nurse Applicant under direction/supervision of Staff Registered Nurse
  • Obtain nursing license and registration within state-specific timeframe or 90 days from hire, whichever sooner
  • Adhere to FMS policies, procedures, training, FMCNA Compliance Program, and patient care/administrative policies
  • Support FMCNA mission, vision, values, customer service philosophy, Quality Enhancement Program (QEP), CQI activities, and process improvements

Responsibilities

  • Direct Patient Care Technician’s delivery of safe/effective chronic hemodialysis therapy per facility policies, corporate/state/federal regulations
  • Delegate tasks to direct patient care staff (LVN/LPNs, Patient Care Technicians, Dialysis Assistants)
  • Assess daily patient care needs and develop appropriate assignments
  • Monitor patient care staff for techniques and adherence to policies/procedures
  • Participate in staff training, orientation of new staff, and required staff meetings
  • Ensure educational needs of patients/family met re: ESRD, renal disease, vascular access, dialysis therapy, diet/fluid/medication compliance, adequacy measures (OLC, AMP, UKM), transplant/modality awareness, catheter reduction, lab values
  • Provide safe/effective delivery of dialysis care, implement treatment prescriptions (Na modeling, Ultrafiltration modeling)
  • Assess patient response to hemodialysis, adjust/modify treatment plan, communicate issues to Team Leader/physician
  • Identify/communicate patient-related issues to Team Leader/physician
  • Initiate Initial/Annual Nursing Assessment, ongoing evaluation/documentation of patient care needs per FMC policies
  • Participate in pre-evaluation, initiation, monitoring, termination, access homeostasis, post-evaluation of hemodialysis per procedures
  • Intervene for changes in patient adequacy status, troubleshoot access flow issues (OLC/AMP)
  • Provide/supervise/monitor hemodialysis access care per procedures
  • Implement/administer/monitor/document patient response to interdialytic transfusions, notify physician of adverse reactions
