Requirements
- Graduation from a Master’s program in social work accredited by the Council on Social Work Education
- Active license as clinical social worker by the Board of Behavioral Sciences
- If residing in Oregon or Washington, an LCSW or AWS is required
- Minimum of one (1) year of experience in healthcare, with preference given to candidates with home health care experience
- Excellent verbal and written communication skills, along with strong organizational abilities
- Current BLS, hands-on CPR certification
- Valid driver's license and reliable, insured transportation
Responsibilities
- Conduct psychosocial assessments of patients to understand their needs within the context of their illness and environment
- Develop and implement tailored intervention plans based on assessment findings
- Maintain accurate clinical records for all patients referred to social work services
- Offer information and referral services to patients, families, and caregivers regarding practical and environmental needs
- Serve as a liaison between patients, families, caregivers, and community agencies to facilitate access to support services
