Requirements

  • Graduation from a Master’s program in social work accredited by the Council on Social Work Education
  • Active license as clinical social worker by the Board of Behavioral Sciences
  • If residing in Oregon or Washington, an LCSW or AWS is required
  • Minimum of one (1) year of experience in healthcare, with preference given to candidates with home health care experience
  • Excellent verbal and written communication skills, along with strong organizational abilities
  • Current BLS, hands-on CPR certification
  • Valid driver's license and reliable, insured transportation

Responsibilities

  • Conduct psychosocial assessments of patients to understand their needs within the context of their illness and environment
  • Develop and implement tailored intervention plans based on assessment findings
  • Maintain accurate clinical records for all patients referred to social work services
  • Offer information and referral services to patients, families, and caregivers regarding practical and environmental needs
  • Serve as a liaison between patients, families, caregivers, and community agencies to facilitate access to support services
