Requirements

Master's degree in Social Work (MSW) - entry level role
Current state licensure as a Master Social Worker (MSW) where applicable
Knowledge of social work theory of human behavior and accepted methods of social work practice
Familiarity with Chronic Kidney Disease's emotional, psychological, and behavioral impacts
Ability to utilize FKC patient education programs, social work interventions, and quality of life measurement instruments
Knowledge of local vocational/educational rehabilitation programs, community resources, advance directives, and DNR discussions
Adherence to FMCNA Social Work Policy and Measuring Patient Physical and Mental Function Policy

Responsibilities

Assess patients' psychosocial status, strengths, and needs as part of the interdisciplinary team's comprehensive patient assessment
Participate in care planning with the patient and healthcare team to identify interventions for rehabilitation, treatment goals, and quality of life improvement
Provide monitoring and interventions to help patients adjust to dialysis and achieve optimal psychosocial status and quality of life
Offer supportive counseling services within scope of clinical training and state license
Provide educational and goal-directed counseling to transplant-seeking patients
Assist with referrals to community resources (e.g., home health, vocational rehabilitation) and facilitate access to vocational/educational programs
Assess patient awareness of advance directives, provide forms/information, and facilitate discussions with team and family
Participate in discussions of patient DNR status with physician, nurse, patient, and family
Document MSW interactions and interventions
Provide psychosocial support and Social Work Focused Interventions for non-adherence, quality outcomes, and quality of life concerns
Participate in monthly Quality review meetings, reporting on indicators like adherence, Missed/Shortened Treatments, Quality of Life Trends, and Service Recovery
Assess patient knowledge of kidney disease, identify barriers to adherence, and provide tailored education with patient, family, and team
Collaborate with interdisciplinary team to promote positive adjustment, rehabilitation, and quality of life
Educate and support staff on emotional, psychological, and behavioral impacts of Chronic Kidney Disease
Support FKC Quality Indicators, Outcomes, QAI Activities, patient satisfaction, and process improvement for quality goals
