Requirements
- Master's degree in Social Work (MSW) - entry level role
- Current state license as a Master Social Worker (MSW) where applicable
- Knowledge of social work theory of human behavior and accepted methods of social work practice
- Familiarity with FKC patient education programs, social work focused interventions, and quality of life measurement instruments
- Knowledge of FMCNA Social Work Policy and Measuring Patient Physical and Mental Function Policy
- Current knowledge of local vocational/educational rehabilitation programs and community resources (e.g., home health services, vocational rehabilitation)
- Ability to work as part of an interdisciplinary healthcare team
Responsibilities
- Assess patients' psychosocial status, strengths, and areas of need as part of comprehensive patient assessment
- Participate in care planning with the patient and healthcare team to identify interventions for rehabilitation, treatment goals, and quality of life improvement
- Provide monitoring and interventions to help patients adjust to dialysis and achieve optimal psychosocial status and quality of life
- Offer supportive counseling services to patients within scope of clinical training and state license
- Provide educational and goal-directed counseling to patients seeking transplant
- Assist with referrals to community resources (e.g., home health, vocational rehabilitation) to facilitate optimal treatment outcomes
- Help patients access vocational rehabilitation to maintain, obtain, or pursue employment/education
- Assess patient awareness of advance directives, facilitate access to forms/information, and discuss wishes with team, patient, and family
- Participate in discussions of patient DNR status with physician, nurse, patient, and family
- Document MSW interactions and interventions provided to patient and/or family
- Provide psychosocial support and/or Social Work Focused Interventions for non-adherence, quality outcomes, and quality of life concerns based on acuity
- Participate in monthly Quality review meetings, reporting on indicators like adherence (Missed/Shortened Treatments), Quality of Life Trends, and Service Recovery
- Assess patient knowledge of kidney disease for barriers to treatment adherence
- Provide tailored education to patients, families, and team on kidney disease, addressing learning styles, communication barriers, and needs
- Support staff understanding of emotional, psychological, and behavioral impacts of Chronic Kidney Disease on patients and families
- Actively participate in QAI activities, process improvements, and FKC Quality Goals related to patient satisfaction and quality of life
