Requirements

  • Master's degree in Social Work (MSW)
  • Appropriate state licensure as a Social Worker (or pre-licensed where permitted)
  • Clinical training permitting supportive counseling services within scope of practice

Responsibilities

  • Assess patients' psychosocial status, strengths, and areas of need affecting rehabilitation and treatment outcomes as part of comprehensive patient assessment
  • Collaborate with patient and healthcare team to identify effective interventions for rehabilitation, treatment goals, and improved quality of life
  • Utilize patient education programs, social work theory/methods, interventions, and quality of life instruments in assessment and care planning to address barriers and meet goals
  • Provide monitoring and interventions to help patients adjust to dialysis and achieve optimal psychosocial status and quality of life
  • Assess patient knowledge of kidney disease for barriers to treatment adherence
  • Provide supportive counseling services to patients as permitted by clinical training and state license
  • Provide information and assist team/patient with referrals to community resources (e.g., home health, vocational rehabilitation) for optimal outcomes
  • Maintain knowledge of local vocational/educational rehabilitation programs and assist patients with referrals/access to enable employment or education
  • Assess patient awareness of advance directives; assist with accessing forms/information and facilitate discussions with team, patient, family/support persons
  • Participate in discussions of patient DNR status to ensure informed decisions
  • Report on quality indicators
