Requirements

  • Master's degree in Social Work (MSW) - entry level role
  • Ability to provide psychosocial services utilizing Social Work Theory of Human Behavior and accepted methods of social work practice
  • Knowledge of emotional, psychological, and behavioral impact of Chronic Kidney Disease on patients and families
  • Ability to work collaboratively with interdisciplinary healthcare teams
  • Understanding of Quality Indicators, Outcomes, Quality Assessment and Improvement (QAI) activities, and process improvement related to patient satisfaction and quality of life

Responsibilities

  • Provide psychosocial services to in-center and home dialysis patients (if applicable)
  • Assess patients' psychosocial status, strengths, and areas of need as part of the comprehensive patient assessment
  • Participate in care planning with the patient and healthcare team to identify effective interventions for rehabilitation, treatment goals, and improved quality of life
  • Utilize FKC patient education programs, social work theory, methods, interventions, and quality of life measurement instruments in assessment and care planning
  • Provide monitoring and interventions to help patients adjust to dialysis and achieve optimal psychosocial status and quality of life
  • Inform, educate, and support clinic staff on the emotional, psychological, and behavioral impact of Chronic Kidney Disease
  • Actively participate in process improvement activities to support FKC Quality Goals, including patient satisfaction and quality of life
