Requirements

Master's degree in Social Work (MSW) - entry level role
Ability to provide psychosocial services utilizing Social Work Theory of Human Behavior and accepted methods of social work practice
Knowledge of emotional, psychological, and behavioral impact of Chronic Kidney Disease on patients and families
Ability to work collaboratively with interdisciplinary healthcare teams
Understanding of Quality Indicators, Outcomes, Quality Assessment and Improvement (QAI) activities, and process improvement related to patient satisfaction and quality of life

Responsibilities

Provide psychosocial services to in-center and home dialysis patients (if applicable)
Assess patients' psychosocial status, strengths, and areas of need as part of the comprehensive patient assessment
Participate in care planning with the patient and healthcare team to identify effective interventions for rehabilitation, treatment goals, and improved quality of life
Utilize FKC patient education programs, social work theory, methods, interventions, and quality of life measurement instruments in assessment and care planning
Provide monitoring and interventions to help patients adjust to dialysis and achieve optimal psychosocial status and quality of life
Inform, educate, and support clinic staff on the emotional, psychological, and behavioral impact of Chronic Kidney Disease
Actively participate in process improvement activities to support FKC Quality Goals, including patient satisfaction and quality of life
