Requirements
- Master of Social Work (MSW) degree (entry level role)
- Current state license as a social worker (as applicable for counseling scope)
- Knowledge of social work theory of human behavior and accepted methods of social work practice
- Knowledgeable of FMCNA Social Work Policy and Measuring Patient Physical and Mental Function Policy
- Current knowledge regarding local vocational/educational rehabilitation programs
- Ability to assess psychosocial status, strengths, needs, and barriers affecting rehabilitation and treatment outcomes
- Ability to work collaboratively with interdisciplinary healthcare teams
Responsibilities
- Assess patients' psychosocial status, strengths, and areas of need as part of comprehensive patient assessment
- Participate in care planning with patient and healthcare team to identify interventions for rehabilitation, treatment goals, and quality of life improvement
- Utilize FKC patient education programs, social work theory, interventions, and quality of life instruments in assessment and care planning
- Provide monitoring and interventions to help patients adjust to dialysis and achieve optimal psychosocial status and quality of life
- Provide supportive counseling services to patients within scope of clinical training and state license
- Provide educational and goal-directed counseling to patients seeking transplant
- Provide information and assist with referrals to community resources (e.g., home health, vocational rehabilitation)
- Assist patients with referral and access to vocational rehabilitation to support employment or education
- Assess patient awareness of advance directives, assist with accessing forms/information, and facilitate discussions
- Participate in discussions of patient DNR status with physician, nurse, patient, and family
- Document MSW interactions and interventions provided to patient and/or family
- Provide psychosocial support and/or social work-focused interventions for non-adherence, quality outcomes, and quality of life concerns
- Participate in monthly Quality review meetings and report on quality indicators (e.g., adherence, missed/shortened treatments, quality of life trends, service recovery)
- Assess patient knowledge of kidney disease and barriers to adherence; provide tailored education to patient, family, and healthcare team
