Requirements

Master's degree in Social Work (MSW) - entry level role
Current state license as a Master Social Worker (as permitted within scope of clinical training and state license)
Knowledge of Social Work Theory of Human Behavior and accepted methods of social work practice
Knowledgeable of FMCNA/FKC Social Work Policy and Measuring Patient Physical and Mental Function Policy
Current knowledge regarding local vocational/educational rehabilitation programs
Ability to assess psychosocial status, strengths, and areas of need affecting rehabilitation and treatment outcomes
Ability to work collaboratively with interdisciplinary healthcare teams, patients, and families

Responsibilities

Assess patients' psychosocial status, strengths, and areas of need as part of comprehensive patient assessment
Participate in care planning with the patient and healthcare team to identify interventions for rehabilitation, treatment goals, and quality of life improvement
Utilize FKC patient education programs, social work theory, methods, interventions, and quality of life instruments in assessment and care planning
Provide monitoring and interventions to help patients adjust to dialysis and achieve optimal psychosocial status and quality of life
Provide supportive counseling services to patients within scope of clinical training and state license
Provide educational and goal-directed counseling to patients seeking transplant
Provide information and assist with referrals to community resources (e.g., home health services, vocational rehabilitation)
Assist patients with referral and access to vocational rehabilitation to support employment or education
Assess patient awareness of advance directives, assist with accessing forms/information, and facilitate discussions with healthcare team and family/support persons
Participate in discussions of patient DNR status with physician, nurse, patient, and family to ensure informed decisions
Document MSW interactions and interventions provided to patient and/or family
Provide psychosocial support and/or Social Work Focused Interventions for non-adherence, quality outcomes, and quality of life concerns based on acuity level
Participate in monthly Quality review meetings, reporting on indicators like adherence (Missed/Shortened Treatments), Quality of Life Trends, and Service Recovery
Assess patient knowledge of kidney disease for barriers affecting treatment adherence
Provide tailored education to patients, families, and healthcare team on kidney disease, addressing learning styles, communication barriers, and needs
Support FKC Quality Indicators, Outcomes, QAI Activities, patient satisfaction, and process improvement to achieve Quality Goals
Inform, educate, and support staff on emotional, psychological, and behavioral impacts of Chronic Kidney Disease on patients and families
