This is a field-based role where associates primarily operate in the field, traveling to client sites or designated locations as required, with occasional office attendance for meetings or training.
The role requires an RN license to manage member cases, perform telephonic or face-to-face clinical assessments, develop and revise care plans, supervise non-RN clinicians, and coordinate care across physical health, behavioral health, social services, and long-term supports.
The RN provides supervision and direction to non-RN clinicians such as LPN/LVN, LSW, LCSW, LMSW, and other licensed professionals, while interfacing with Medical Directors, Physician Advisors, and Inter-Disciplinary Teams.
A strong candidate is a licensed RN with experience in case management, clinical assessments, care planning for members with chronic illnesses or disabilities, and supervising non-RN clinicians to optimize health care across the continuum.
