  • LPN/LVN, LSW, LCSW, LMSW, or other license other than RN in accordance with applicable state law
  • Nursing Diploma, AS in Nursing, or related field
  • Minimum of 2 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or similar role (or equivalent combination of education and experience)
  • Current, unrestricted LPN/LVN, LSW, LCSW, LMSW, or other applicable license other than RN in state(s) of practice
  • Must be within reasonable commuting distance from posting location(s) near Columbus, OH (unless accommodation granted)

Responsibilities

  • Work under the direction/supervision of an RN with overall responsibility for the member's case
  • Contribute to LTSS care coordination within scope of licensure, including assisting RN with telephonic or face-to-face assessments for identification, evaluation, coordination, and management of member's physical health, behavioral health, social services, and long-term services/supports needs
  • Assist responsible RN in identifying members for high-risk complications
  • Obtain clinical data as directed by the responsible RN
  • Assist responsible RN in identifying members that would benefit from alternative level of care or other waiver programs
  • Provide all collected information to the responsible RN for verification, interpretation, additional assessments, care plan development, monitoring, evaluation, and revision
  • Participate in coordinating care for members with chronic illnesses, co-morbidities, and/or disabilities as directed by RN, in conjunction with RN, member, and health care team, to ensure cost-effective and efficient utilization of health benefits (using decision-making based on member needs, disease processes, terminology, and clinical guidelines, without nursing judgment)
