LPN/LVN, LSW, LCSW, LMSW, or other license other than RN in accordance with applicable state law
Nursing Diploma, AS in Nursing, or related field
Minimum of 2 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or similar role (or equivalent combination of education and experience)
Current, unrestricted LPN/LVN, LSW, LCSW, LMSW, or other applicable license other than RN in state(s) of practice
Must be within reasonable commuting distance from posting location(s) near Columbus, OH (unless accommodation granted)

Responsibilities

Work under the direction/supervision of an RN with overall responsibility for the member's case
Contribute to LTSS care coordination within scope of licensure, including assisting RN with telephonic or face-to-face assessments for identification, evaluation, coordination, and management of member's physical health, behavioral health, social services, and long-term services/supports needs
Assist responsible RN in identifying members for high-risk complications
Obtain clinical data as directed by the responsible RN
Assist responsible RN in identifying members that would benefit from alternative level of care or other waiver programs
Provide all collected information to the responsible RN for verification, interpretation, additional assessments, care plan development, monitoring, evaluation, and revision
Participate in coordinating care for members with chronic illnesses, co-morbidities, and/or disabilities as directed by RN, in conjunction with RN, member, and health care team, to ensure cost-effective and efficient utilization of health benefits (using decision-making based on member needs, disease processes, terminology, and clinical guidelines, without nursing judgment)
