Responsibilities

The Licensed Psychiatric & Family Nurse Practitioner will deliver patient care services through a remote technology platform, working in collaboration with a dedicated team of professionals. They will independently provide holistic, evidenced-based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, psychiatric diagnosis and prescribing, mental health counseling and disposition planning for patients ranging in age 18 months and above. This includes education and treatment for pregnancy prevention, STI Prevention and safer sex practices. They will serve as a clinical expert and resource for clinical quality initiatives, develop program insight, provide process improvement, and provide oversight of behavioral health care, including quality and training to new providers. They will evaluate primary care, acute, chronically ill, and transitional care patients, provide patient counseling, and document all patient care within an EHR. The provider will also effectively work within a patient care team, coordinate care with internal and external colleagues, and advocate for their patients and their practice.
