Requirements

  • Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) licensure
  • Adherence to state nurse practice laws, physician orders, company policies, procedures, and regulatory requirements
  • Knowledge in operation of dialysis-related equipment (hemodialysis machines, water treatment systems, peritoneal cyclers, peritoneal assist devices)
  • Current knowledge of dialysis principles and techniques through in-service classes
  • Ability to perform physical demands of the role (implied by physical demands section)

Responsibilities

  • Provide direct patient care under the supervision of a Registered Nurse, including administering intravenous medications
  • Assist in identification, evaluation, and training of home dialysis candidates
  • Document patient training, treatment, and related information accurately in patient records and Medical Information System
  • Conduct ongoing monitoring, assessment, and intervention for home patients (home visits, clinical appointments, training sessions, lab studies, annual studies like x-ray and EKG)
  • Respond to problems or emergencies and ensure resolution
  • Ensure compliance with patient supply procedures, documentation, and maintenance of durable medical equipment (preventative maintenance, water analysis)
  • Perform monthly chart audits and report results to RN/Coordinator or Manager
  • Monitor Peritonitis Rate and PET test results; report to physicians, Quality Assurance Committee, RN/Coordinator, or Manager
  • Collaborate with physician, RN/Coordinator, or Manager to adjust treatment plans and schedule back-up treatments
  • Educate pre-dialysis and existing ESRD patients on treatment options
  • Prepare home supply inventory packages and monitor efficient use of supplies
  • Troubleshoot equipment problems via telephone and ensure resolution
  • Address blood spills per FMCNA Bloodborne Pathogen Control Policies
  • Participate in staff meetings, patient care plan meetings, and educational in-services
  • Act as a resource for other staff and train facility staff on FMS Customer Service Model
  • Drive FMS culture through values and customer service standards; maintain effective relationships and resolve issues timely
  • Other duties as assigned
