Requirements

  • Licensed Practical Nurse (LPN) - active license in the state of practice from the Board of Nursing upon hire
  • Visual acuity adequate to perform position duties
  • Ability to communicate effectively with others, hear, understand, and distinguish speech and other sounds
  • Preferred: Less than 1 year experience in Long Term Care

Responsibilities

  • Perform all licensed practical nursing functions and services as directed by the Registered Nurse to all residents
  • Complete assessments on residents, make appropriate interventions, apply knowledge, prioritize resident care, and use the nursing process (assess, plan, implement, evaluate)
  • Comply with all pharmaceutical policies and procedures; ensure medications/treatments are given safely and accurately per physician’s orders; administer medications according to facility policy and be competent in Pyxis medication administration system
  • Participate in the care planning process; assure resident care is implemented consistently and appropriate to the age of those served; initiate and update plan of care as appropriate
  • Comply with standards of professional documentation
  • Communicate and manage the delivery of resident care; ensure quality care is provided
  • Guide staff in meeting residents’ needs, practicing routines based on residents’ needs
  • Participate in physician rounds; communicate changes in resident condition to physician using SBAR communication tool; process and transcribe physician’s orders and implement according to facility policy
  • Seek educational opportunities, share knowledge with others, and assist with orientation of new staff
  • Investigate incidents and patient/family complaints; make immediate report to VP of Resident Care or Administrator on-call; facilitate conflict management while respecting diversity
