Responsibilities

  • Serve as primary interface to referral sources (physicians, hospital SWs, discharge planners, RNs, acute care staff)
  • Obtain complete and accurate demographic/medical information from patient records
  • Complete initial, comprehensive, and ongoing assessments of patient/family needs, including physical assessment and illness history
  • Introduce agency services, explain scope, coordinate start-of-care visits
  • Order DME, supplies, and medications for hospice care
  • Deliver community education to assisted living, skilled nursing facilities, and hospitals
  • Provide bedside patient education on hospice, disease progression, and end-of-life care
  • Attend discharge planning meetings to ensure safe transitions home
  • Determine hospice eligibility and appropriate level of care
  • Communicate with clinical team on complex patients using Liaison Coordination notes to prevent re-hospitalizations
  • Assist in obtaining regulatory documentation
  • Provide professional nursing care using nursing process elements
  • Regularly re-evaluate patient/family needs and revise plan of care
  • Develop care plan with goals, nursing diagnosis, palliative actions, including patient/family
  • Counsel patient/family, provide health care instructions, teach self-care techniques
  • Complete, maintain, submit clinical notes on patient condition, pain/symptom management
  • Communicate with physicians on needs/changes, obtain orders
  • Coordinate with community health persons and interdisciplinary group
  • Provide medication, diet instructions, health counseling
  • Maintain safe environment, ensure continuity of care
  • Follow up with doctor's office, clinical/management staff on concerns
  • Participate in on-call duties
  • Supervise ancillary personnel and delegate responsibilities
