Responsibilities

  • Administer skilled nursing care to patients on an intermittent basis in their place of residence in accordance with physician orders and plan of care
  • Make the initial evaluation visit and regularly reevaluate the patient’s nursing needs
  • Develop, initiate, and revise the Plan of Care (POC) as necessary
  • Provide services requiring substantial specialized nursing skills
  • Initiate appropriate preventive and rehabilitative nursing procedures
  • Prepare clinical and progress notes for each patient visit and summaries of care conferences in a timely manner
  • Coordinate services and inform personnel of changes in the patient’s condition and needs
  • Counsel the patient and family/significant others in meeting nursing and related needs
  • Participate in and present in-service programs
  • Adhere to established Agency policies and procedures, including infection control and universal precautions
  • Process orders and notify physician of patient needs and changes in condition
  • Complete certification/recertification orders and discharge summaries
  • Determine the amount and type of nursing needed by each individual patient
  • Refer patients to Physical Therapist, Speech Language Pathologist, Occupational Therapist, and Medical Social Worker as required
  • Develop Home Health Aide Care Plan and perform supervisory visits for HHA and LPN/LVN as required
  • Give total patient care as needed
  • Take on-call duty nights, weekends, and holidays as assigned
  • Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition per Agency time frames
  • Appropriately utilize ICD-10 codes
  • Complete appropriate documentation in a timely manner for compliance
  • Demonstrate commitment to professional growth through in-service programs and maintaining competency
  • Maintain highest standards of professional conduct regarding confidential information
  • Maintain required licenses, certifications, and mandatory skill updates
  • Conduct patient care conferences on assigned patients
  • Participate in peer review and Quality Assurance and Performance Improvement as assigned
