Requirements

  • Home Health Aide Certification-NJ Board required
  • Valid, unrestricted driver’s license required
  • Successful completion of State Certified Home Health Aide course
  • High School Diploma or equivalency preferred
  • Home Health Aide or Nurse Aide experience preferred

Responsibilities

  • Knowledgeable of and supports the plan of care under the direction of the RN
  • Performs personal care and household chores to/for the patient as established in the plan of care in order to maintain patient’s health status (including bathing, dressing, feeding, assisting with mobility using assistive devices if necessary, bed making, laundry, shopping, and meal preparation)
  • Submits all documentation (care plan, time sheets, API, etc) accurately and timely
  • Identifies and communicates changes in the patient’s status to RN and responds appropriately
  • Reinforces education of the patient and family / responsible party regarding individual care needs, wellness, and safety issues
  • Practices standard precautions and patient safety principles
  • Identifies and participates in performance improvement activities. Maintains departmental level competencies
