  • Entry level designation into RN CAP 2 or advancement from RN CAP 1
  • Accountability for provision and coordination of clinically competent care including assessment, planning, intervention, and evaluation for assigned patients
  • Ability to delegate tasks to direct patient care staff (RNs, LVN/LPNs, Patient Care Technicians)
  • Participation in decision-making, teaching, leadership, and quality improvement activities
  • Compliance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state, and federal regulations
  • Ability to perform under direction of Supervisor and with guidance from Clinical Educator, Preceptor, or in collaboration with other RNs
  • Capability for ongoing, systematic collection and analysis of patient data pre-during-post hemodialysis treatment
  • Skills in assessing, collaborating, and documenting patient/family learning needs for education
  • Ability to direct and provide safe and effective hemodialysis therapy in collaboration with staff
  • Competence in administering medications per prescription or approved algorithms with documentation
  • Ability to initiate or assist with emergency response measures
  • Role as resource, leader, coach, mentor, and role model for employees
  • Knowledge of correct laboratory collection, processing, and shipping procedures
  • Ability to identify expected outcomes, document, and update nursing assessment and plan of care via Interdisciplinary Team collaboration
  • Ability to ensure patient awareness of transplant and treatment modality options
  • Participation in education and quality improvement projects
  • Completion of CAP requirements to maintain or advance
  • Physical ability for heavy lifting, moving patients, assisting with ambulation, frequent prolonged standing, bending, and moving equipment up to 200 lbs. (with assistance) and lifting solutions up to 30 lbs. as high as 5 feet
  • Tolerance for health care facility work environment with air temperature control and moderate noise

Responsibilities

  • Perform ongoing, systematic collection and analysis of patient data pre-during-post hemodialysis treatment for assigned patients and document in medical record
  • Make adjustments or modifications to treatment plan as indicated and notify Team Leader, Charge Nurse, Supervisor, or Physician as needed
  • Assess, collaborate, and document patient/family’s basic learning needs to provide initial and ongoing education
  • Direct and provide, in collaboration with direct and ancillary patient care staff, all aspects of daily safe and effective hemodialysis therapy delivery to assigned patients
  • Administer medications as prescribed or per approved algorithm(s) and document justification and effectiveness
  • Initiate or assist with emergency response measures
  • Serve as resource, leader, coach, mentor, and role model for new and incumbent employees
  • Ensure correct laboratory collection, processing, shipping procedures, and reschedule missed or insufficient collections
  • Identify expected outcomes, document, and update nursing assessment and plan of care for assigned patients through Interdisciplinary Team collaboration
  • Ensure patient awareness related to transplant and treatment modality options
  • Participate in education and quality improvement projects at facility and area level as directed by Supervisor
  • Serve as Preceptor to new employees as needed
  • Perform all other duties as assigned by Supervisor
