Requirements
- Registered Nurse with a minimum BSN
- Residing in a compact state with a current, active, unencumbered compact RN license with multi-state privileges
- 4+ years recent experience in a hospital and/or healthcare practice serving maternal-child health populations (minimum 1 year in NICU and 1 year in labor and delivery/postpartum or Antepartum)
- Growth mindset with ability to approach process change and ambiguous situations with enthusiasm, creativity, and accountability
- Eager to thrive in a fast-paced, metric-driven environment
- Internet-connected and able to work remotely via video, phone, and text
- Willing to travel occasionally (infrequent)
- Willing to work weekends and holidays (rotating schedule, M-F 5p-12a EST with rotating weekend and holiday coverage)
- Understand the prevalence and role of birth inequity and structural racism in maternal health
Responsibilities
- Coordinate holistic family support by connecting parents to specialists, resource partners, and pediatric providers for NICU journey and safe discharge
- Conduct ongoing clinical and psychosocial assessment of new patients, providing reassurance and building rapport
- Establish individualized care plans based on evidence-based standards to meet specific health needs
- Develop wellness goals by determining motivating factors for best health outcomes
- Identify and address barriers to care identified by patients and/or the care team
- Reduce care gaps (e.g., missed appointments, medication management) through frequent patient engagements
- Assess need for and educate patients on remote health monitoring equipment
- Provide ongoing surveillance and timely response to abnormal diagnostic results (labs, radiology, etc.)
- Assess urgent concerns and proactively triage patients to support appropriate emergency service utilization
- Document all care/interactions timely and escalate to multidisciplinary teams as needed
- Support program and product development by providing user feedback
