Registered Nurse (RN) licensure
Entry-level designation into the Clinical Advancement Program (CAP); required to complete CAP requirements to advance
Ability to perform under the direction of the Supervisor with guidance from the Educator, Preceptor, or in collaboration with another Registered Nurse
Capability to handle physical demands: heavy lifting, moving equipment and patients, assisting with ambulation, frequent prolonged standing, bending over, moving machines/equipment up to 200 lbs. (with assistance), lifting chemical/water solutions up to 30 lbs. as high as 5 ft

Responsibilities

Perform ongoing, systematic collection and analysis of dialysis data for assigned patients, document in medical record, make adjustments to treatment plan as indicated, and notify Supervisor or physician as needed
Assess, collaborate, and document patient/family’s basic learning needs; provide initial and ongoing education to patients and family
Direct and provide, in collaboration with the patient, home care partner, direct and ancillary staff, all aspects of safe and effective delivery of dialysis therapy to assigned patients
Administer medications as prescribed or per approved algorithm(s), and document medical justification and effectiveness
Initiate or assist with emergency response measures
Serve as a resource for health care team; participate in staff training and orientation of new staff as assigned
Ensure correct laboratory collection, processing, and shipping procedures; reschedule missed or insufficient collections
Identify expected outcomes, document and update nursing assessment and plan of care through collaboration with the Interdisciplinary Team
Ensure patient awareness of transplant and treatment modality options
Assist in identification, evaluation, selection, and education of Home Dialysis candidates and Home Partners
Perform assessment of Home Dialysis candidate’s home environment, and partner/family readiness and ability to perform dialysis treatments
Train Home Dialysis patients and/or Home Partners on safe, effective operation and maintenance of all Home Dialysis equipment and supplies through an organized formal training program
Participate in decision-making, teaching, leadership functions, and quality improvement activities
Perform all other duties as assigned by Supervisor
Comply with Fresenius Kidney Care policy, procedures, standards of nursing practice, state and federal regulations
