Requirements

  • Registered Nurse (RN) license
  • Entry-level designation into Clinical Advancement Program (CAP 1); required to complete CAP requirements to advance
  • Ability to perform under direction of Supervisor with guidance from Educator, Preceptor, or in collaboration with another RN
  • Capability to handle physical demands: heavy lifting, moving equipment/patients up to 200 lbs. (with assistance), frequent prolonged standing, bending, lifting chemical/water solutions up to 30 lbs. as high as 5 ft

Responsibilities

  • Perform ongoing, systematic collection and analysis of dialysis data for assigned patients, document in medical record, adjust/modify treatment plan as indicated, and notify Supervisor or physician as needed
  • Assess, collaborate, and document patient/family’s basic learning needs; provide initial and ongoing education to patients and family
  • Direct and provide, in collaboration with patient, home care partner, direct/ancillary staff, all aspects of safe/effective delivery of dialysis therapy to assigned patients
  • Administer medications as prescribed or per approved algorithm(s), document medical justification and effectiveness
  • Initiate or assist with emergency response measures
  • Serve as resource for health care team; participate in staff training and orientation of new staff as assigned
  • Ensure correct laboratory collection, processing, shipping; reschedule missed/insufficient collections
  • Identify expected outcomes; document/update nursing assessment and plan of care through collaboration with Interdisciplinary Team
  • Ensure patient awareness of transplant and treatment modality options
  • Assist in identification, evaluation, selection, and education of Home Dialysis candidates and Home Partners
  • Perform assessment of Home Dialysis candidate’s home environment, partner/family readiness, and ability to perform treatments
  • Train Home Dialysis patients/Home Partners on safe, effective operation/maintenance of all Home Dialysis equipment/supplies via organized formal training program
  • Participate in decision-making, teaching, leadership functions, and quality improvement activities
  • Perform all other duties as assigned by Supervisor
  • Comply with Fresenius Kidney Care policy, procedures, standards of nursing practice, state/federal regulations
