Requirements

Strong interdisciplinary collaboration and communication skills via EMR, huddles, and meetings

Responsibilities

Provide comprehensive medication management (CMM) for patients with chronic conditions
Conduct telehealth (or face-to-face) visits to assess medication efficacy, safety, adherence, and understanding
Develop individualized care plans with interdisciplinary teams (physicians, advanced practice providers, nurses, care coordinators)
Provide patient education on disease states, medications, and self-management strategies
Ensure appropriateness of drug therapy, correct dosing, and selection of cost-effective medications
Interpret laboratory and patient-specific data to develop treatment plans and monitor therapy response
Issue recommendations to medical teams on therapies or alternatives based on literature and experience
Manage patients under collaborative practice agreements (where permissible), including medication initiation, adjustment, and discontinuation
Identify and engage high-risk or high-cost patients using population health tools and reports
Participate in quality improvement initiatives for chronic disease metrics (e.g., A1c, blood pressure, adherence)
Collaborate to reduce hospital readmissions and ED utilization
Develop and maintain clinical protocols, workflows, and documentation templates
Track and report clinical outcomes, intervention metrics, and quality measures
Serve as medication expert on primary care team, providing evidence-based recommendations
Communicate interventions and care plans via EMR, team huddles, and meetings
Participate in care team meetings and provider education sessions
