Requirements

  • Experience issuing recommendations based on medical literature and clinical experience
  • Ability to manage patients under collaborative practice agreements (where permissible), including initiation, adjustment, and discontinuation of medications
  • Familiarity with population health tools, risk stratification, and EMR dashboards
  • Participation in quality improvement initiatives and tracking clinical outcomes
  • Strong interdisciplinary collaboration and communication skills via EMR, huddles, and meetings

Responsibilities

  • Provide comprehensive medication management (CMM) for patients with chronic conditions
  • Conduct telehealth (or face-to-face) visits to assess medication efficacy, safety, adherence, and understanding
  • Develop individualized care plans with interdisciplinary teams (physicians, advanced practice providers, nurses, care coordinators)
  • Provide patient education on disease states, medications, and self-management strategies
  • Ensure appropriateness of drug therapy, correct dosing, and selection of cost-effective medications
  • Interpret laboratory and patient-specific data to develop treatment plans and monitor therapy response
  • Issue recommendations to medical teams on therapies or alternatives based on literature and experience
  • Manage patients under collaborative practice agreements (where permissible), including medication initiation, adjustment, and discontinuation
  • Identify and engage high-risk or high-cost patients using population health tools and reports
  • Participate in quality improvement initiatives for chronic disease metrics (e.g., A1c, blood pressure, adherence)
  • Collaborate to reduce hospital readmissions and ED utilization
  • Develop and maintain clinical protocols, workflows, and documentation templates
  • Track and report clinical outcomes, intervention metrics, and quality measures
  • Serve as medication expert on primary care team, providing evidence-based recommendations
  • Communicate interventions and care plans via EMR, team huddles, and meetings
  • Participate in care team meetings and provider education sessions
