This is a field-based position requiring residence in McHenry County IL or surrounding areas (specific zip codes: 60013, 60014, 60050, 60098, 60102, 60110, 60142, 60152, 60156, 60614), with 50-75% travel for face-to-face member meetings in Northside Chicago/loop and surrounding areas. Reliable transportation is required, and mileage is reimbursed per company policy.
Candidates need an active IL BH clinical license (LCSW, LCPC, LMFT), a Master's degree in social services, human services or behavioral health, minimum 2 years of experience in behavioral health or social services, and residence in specified areas with travel willingness.
Preferred experience includes case management and discharge planning, managed care, Microsoft Office, and working with HIV and AIDS populations.
This is a full-time position with anticipated weekly hours of 40.
