Case Manager Registered Nurse – Louisiana Field at CVS Health

The typical pay range is $54,095.00 - $116,760.00, depending on factors like experience, education, and geography. This position is also eligible for a CVS Health bonus, commission, or short-term incentive program in addition to the base pay.

This is a field position requiring residence in specific Louisiana parishes and travel 75% or more of the time within West Feliciana, East Feliciana, Pointe Coupee, East Baton Rouge, West Baton Rouge, Iberville, or Ascension parishes. Reliable transportation is required, with mileage reimbursed per company policy.

Candidates must be a Registered Nurse with a current unrestricted Louisiana license, have 3+ years clinical practice experience with adults, 2+ years with MS Office Suite and computer navigation, and 1+ year in case management and discharge planning.
