Requirements

Candidates must possess an active Registered Nurse (R.N.) license with no disciplinary action, and hold an active Compact nursing license residing in the state that holds your compact license. A minimum of an Associate's degree in Nursing and at least 3 years of clinical nursing experience are required, along with demonstrated clinical knowledge in managing chronic conditions and developing care plans. Intermediate to advanced computer skills for navigating multiple systems are necessary, and bilingual fluency in Spanish and English, demonstrated by passing a language test, is mandatory.

Responsibilities

The Care Manager will telephonically assess members, create actionable care plans, and coordinate care for acute situations and transitions of care, including safe discharge planning. They will guide members and families toward appropriate resources and assess physical, environmental, and psycho-social health issues. Responsibilities include collaborating with a multidisciplinary team to manage health issues and provide timely interventions, while meeting daily quality and productivity measures and maintaining a dedicated home office space with reliable internet.
