Candidates must be a board-certified Family Nurse Practitioner or Physician Assistant, licensed in Connecticut, and comfortable practicing the full spectrum of comprehensive care, including pediatrics and mental health. Familiarity with acute care, primary care, and behavioral health is required, as well as the ability to deliver text and video-based care remotely.

- Board certification in Family or Internal Medicine - Active Connecticut state medical license, IMLC preferred - 3+ years of post-residency work experience - Demonstrated experience in Acute Care or Urgent Care settings - Brick-and-mortar primary care and behavioral health experience

- Board-Certified in Family or Internal Medicine - 3+ years of post-residency work experience - A minimum of 2+ years of experience in an in-person primary care setting - Experience with virtual visits required - Must have an active CA state medical license, IMLC preferred - Must have prior experience with EPIC EMR - Must be currently enrolled, or eligible for enrollment, as a Medicare provider - Clean background and medical malpractice history - Willing to commit 40 hours per week, 36 clinical
