Responsibilities

Lead collaboration among interdisciplinary team to develop, coordinate, and facilitate direct social services interventions for assigned patients and support systems
Conduct individual, group, and family therapy sessions
Assess patients' needs to identify mental health concerns, strengths, and challenges
Develop individualized treatment plans and guide implementation of clinical interventions, including facilitating treatment team meetings
Provide counseling and support using evidence-based techniques in sessions and milieu
Educate patients on their conditions, self-care strategies, treatment options, costs/benefits of engagement, and aftercare plans
Document patient progress, goals, and interventions accurately and timely
Collaborate with internal and external professionals to coordinate care
Provide training, supervision, guidance, and support to less experienced therapists and student interns
Report and document suspected cases of physical, sexual abuse, and/or neglect per law and policy
Demonstrate strong interdisciplinary collaboration and professional conduct
Communicate assertively and accurately
Receive feedback productively for development
Support staffing/scheduling changes as needed for program and patient care
Lead clinical initiatives, projects, and activities for system-wide improvement
Ensure patient safety and adherence to regulatory compliance
Perform other duties as directed by manager/clinical operations supervisor
