Requirements

Responsibilities

  • Lead collaboration among interdisciplinary team to develop, coordinate, and facilitate direct social services interventions for assigned patients and support systems
  • Conduct individual, group, and family therapy sessions
  • Assess patients' needs to identify mental health concerns, strengths, and challenges
  • Develop individualized treatment plans and guide implementation of clinical interventions, including facilitating treatment team meetings
  • Provide counseling and support using evidence-based techniques in sessions and milieu
  • Educate patients on their conditions, self-care strategies, treatment options, costs/benefits of engagement, and aftercare plans
  • Document patient progress, goals, and interventions accurately and timely
  • Collaborate with internal and external professionals to coordinate care
  • Provide training, supervision, guidance, and support to less experienced therapists and student interns
  • Report and document suspected cases of physical, sexual abuse, and/or neglect per law and policy
  • Demonstrate strong interdisciplinary collaboration and professional conduct
  • Communicate assertively and accurately
  • Receive feedback productively for development
  • Support staffing/scheduling changes as needed for program and patient care
  • Lead clinical initiatives, projects, and activities for system-wide improvement
  • Ensure patient safety and adherence to regulatory compliance
  • Perform other duties as directed by manager/clinical operations supervisor
