Responsibilities

  • Supports FMCNA's mission, core values, expected behaviors, and customer service philosophy; drives FMS culture through values and customer service standards
  • Provides outstanding customer service to all external and internal customers; develops and maintains effective relationships through timely communication; responds, resolves, and follows up on customer service issues
  • Assesses patients' psychosocial status, strengths, and areas of need as part of the interdisciplinary team's comprehensive patient assessment
  • Participates in care planning with the patient and healthcare team to identify interventions for rehabilitation goals and improved quality of life
  • Provides supportive and goal-directed counseling, including for transplant-seeking patients, treatment adherence, vocational/educational rehabilitation, and quality of life
  • Assesses patient awareness of advance directives; assists with accessing forms/information and facilitates discussions with healthcare team, patient, and family/support persons
  • Provides general information about Do Not Resuscitate Orders and Advanced Directives
  • Refers patients and team to community resources (e.g., home health services, vocational rehabilitation) for optimal treatment outcomes
  • Participates in discussions of patient DNR status with physician and nurse to ensure understanding and informed decision-making
  • Provides usual care and/or Social Work Intensive for non-adherence and quality of life concerns
  • Assesses patient knowledge of kidney disease and barriers to treatment adherence; provides tailored education with patient, family, and healthcare team
  • Informs, educates, and supports staff on the emotional, psychological, and behavioral impact of Chronic Kidney Disease on patients and families
  • Actively participates in Quality Indicators, Outcomes, QAI activities, process improvement, and efforts to achieve FMCNA Quality Goals, including patient satisfaction and quality of life
