Requirements

  • Registered Nurse (RN) with an active license in the state of practice upon hire
  • Preferred: 1-3 years Long Term Care nursing with MDS experience
  • Ability to work day shift (8:00 a.m. - 4:30 p.m.), full time (at least 80 hours per pay period)
  • Visual acuity adequate to perform position duties
  • Ability to communicate effectively, hear, understand, and distinguish speech and other sounds

Responsibilities

  • Coordinate daily resident care, including comprehensive assessments, care conferences, and written plans of care
  • Oversee admission, discharge, and transitions of care
  • Complete quarterly, annual, and significant change observations and assessments in a timely manner
  • Act as resident advocate and provide leadership, organization, and direction to the nursing department
  • Audit resident charts, care plans, and charting to ensure documentation supports care provided and reimbursement level
  • Complete and transmit MDS assessments and reports per regulatory requirements and facility policies
  • Identify and communicate needs for significant change in status assessments
  • Ensure compliance with Medicare, Medicaid, and other benefit program guidelines
  • Maintain working relationships with all disciplines involved in resident care planning
  • Complete Discharge Assessments and entry/re-entry tracking forms per Federal regulations
  • Schedule MDS assessments, maintain monthly calendar, and notify IDT of schedule changes
  • Electronically submit MDS information, review, revise, correct, and resubmit as needed
  • Contribute to interdisciplinary care plans by communicating pertinent resident issues
  • Participate in facility QA initiatives
  • Train as part of the Infection Control team for Long Term Care
  • Follow policies on fire, tornado, evacuation, disaster plans, universal precautions, infection control, and hazardous/chemical communication
  • Mentor new employees and assist with their orientation
  • Lead by example, following CARE standards
