Requirements
- Registered Nurse (RN) license
- Entry-level designation into the Clinical Advancement Program (CAP); required to complete CAP requirements to advance
- Ability to perform under the direction of the Supervisor with guidance from the Educator, Preceptor, or in collaboration with another RN
- Knowledge of Fresenius Kidney Care policy, procedures, standards of nursing practice, state and federal regulations
- Physical ability to provide direct patient care involving heavy lifting, moving equipment/patients, assisting with ambulation, frequent prolonged standing, bending, and occasionally moving machines/equipment up to 200 lbs. or lifting solutions up to 30 lbs
Responsibilities
- Perform ongoing, systematic collection and analysis of dialysis data for assigned patients, document in medical record, make adjustments to treatment plan as indicated, and notify Supervisor or physician as needed
- Assess, collaborate, and document patient/family’s basic learning needs to provide initial and ongoing education
- Direct and provide, in collaboration with the patient, home care partner, direct and ancillary staff, all aspects of safe and effective delivery of dialysis therapy to assigned patients
- Administer medications as prescribed or per approved algorithm(s), and document medical justification and effectiveness
- Initiate or assist with emergency response measures
- Serve as a resource for health care team, participate in staff training and orientation of new staff as assigned
- Ensure correct laboratory collection, processing, shipping procedures, and reschedule missed or insufficient collections
- Identify expected outcomes, document and update nursing assessment and plan of care through collaboration with the Interdisciplinary Team
- Ensure patient awareness of transplant and treatment modality options
- Assist in identification, evaluation, selection, and education of Home Dialysis candidates and Home Partners
- Perform assessment of Home Dialysis candidate’s home environment, partner/family readiness, and ability to perform dialysis treatments
- Train Home Dialysis patients and/or Home Partners on safe, effective operation and maintenance of Home Dialysis equipment and supplies through organized formal training program
- Participate in decision-making, teaching, leadership functions, and quality improvement activities
- Assess and manage patients’ response to home dialysis training and treatment by following protocols and communicate issues to physician
- Perform all other duties as assigned by Supervisor
