- Registered Nurse (RN) license
- Ability to work full-time, 8:00am - 4:30pm Monday to Friday, with light on-call duties
- Willingness to travel to multiple locations (Middletown home clinic, plus Enola, Camp Hill, Newberry as needed)
- Compliance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state, and federal regulations
Responsibilities
- Participate in decision-making, teaching, leadership, and quality improvement activities as a member of the nephrology health care team to enhance patient care outcomes and home therapies program operations
- Identify and evaluate candidates for the Home Therapies Program, including assessment of patients’ home environment if needed
- Perform assessment and identify barriers of the Home Dialysis candidate’s home environment and caretaker readiness and ability to perform dialysis treatments in the home
- Admit new patients to the Home Therapies Program using the appropriate processes
- Coordinate patient care including assessment, planning, intervention, and evaluation for an assigned group of hemodialysis patients
- Assess and manage patients’ response to home dialysis training and treatment therapy by following prescribed predetermined protocols
- Analyze dialysis data for Home Therapy patients and document and adjust or modify the treatment plan as needed
- Provide initial and ongoing education to patient and family on home dialysis treatment and the operation and maintenance of all home therapies equipment
- Ensure patient awareness related to transplant and treatment modality options
- Administer medications as prescribed or in accordance with approved algorithm(s), and document appropriate medical justification and effectiveness
- Initiate or assist with emergency responses
