Registered Nurse (RN) license
Entry-level designation into Clinical Advancement Program (CAP 1); required to complete CAP requirements to advance
Ability to perform physical demands: heavy lifting, moving equipment/patients, assisting with ambulation, frequent prolonged standing, bending, moving machines/equipment up to 200 lbs. (with assistance), lifting chemical/water solutions up to 30 lbs. as high as 5 feet

Responsibilities

Provide and coordinate clinically competent care including assessment, planning, intervention, and evaluation for assigned patients
Assess and manage patients’ response to home dialysis training and treatment therapy following prescribed protocols; communicate patient issues to physician as needed
Perform ongoing, systematic collection and analysis of dialysis data, document in medical record, adjust treatment plans, and notify Supervisor or physician
Assess, collaborate, and document patient/family learning needs; provide initial and ongoing education
Direct and provide safe and effective delivery of dialysis therapy in collaboration with patient, home care partner, and staff
Administer medications as prescribed or per approved algorithms; document justification and effectiveness
Initiate or assist with emergency response measures
Serve as resource for health care team; participate in staff training and orientation
Ensure correct laboratory collection, processing, shipping; reschedule missed collections
Identify expected outcomes, document/update nursing assessment and plan of care via Interdisciplinary Team collaboration
Ensure patient awareness of transplant and treatment modality options
Assist in identification, evaluation, selection, and education of Home Dialysis candidates and Home Partners
Assess home environment barriers and partner/family readiness for home dialysis
Train Home Dialysis patients/Home Partners on safe operation and maintenance of equipment/supplies via formal training program
Participate in decision-making, teaching, leadership, and quality improvement activities
Perform all other duties as assigned by Supervisor
