Requirements

Registered Nurse (RN) licensure
Ability to perform under the direction of the Supervisor and with guidance from the Educator/Preceptor or in collaboration with another RN
Completion of CAP requirements to maintain or advance
Physical ability to provide direct patient care, including heavy lifting, moving patients, assisting with ambulation, frequent prolonged standing (reasonable accommodations for disabilities)
Based in regional Midwest area (Geraldton) as a spoke nurse

Responsibilities

Perform ongoing systematic collection and analysis of dialysis data for assigned patients, document in medical record, adjust/modify treatment plan as indicated, notify Supervisor or physician as needed
Assess, collaborate, and document patient/family’s basic learning needs; provide initial and ongoing education to patients and family
Direct and provide, in collaboration with patient, home care partner, direct and ancillary staff, all aspects of safe and effective delivery of dialysis therapies to assigned patients
Administer medications as prescribed or per approved algorithm(s), document medical justification and effectiveness
Initiate or assist with emergency response measures
Serve as resource, leader, coach, mentor, and role model for new and incumbent employees by setting examples of appropriate behavior, work habits, and attitudes
Ensure correct laboratory collection, processing, and shipping procedures; reschedule missed or insufficient collections
Identify expected outcomes, document and update nursing assessment and plan of care through collaboration with Interdisciplinary Team
Ensure patient awareness of transplant and treatment modality options
Assist in identification, evaluation, selection, and education of Home Dialysis candidates and Home Partners
Perform assessment of Home Dialysis candidate’s home environment, partner/family readiness and ability to perform dialysis treatments
Train home dialysis patients and/or Home Partners on safe, effective operation and maintenance of Home Dialysis equipment and supplies via formal standardized training program
Participate in education and quality improvement projects at facility and area level as directed by Supervisor
Serve as Preceptor to new employees
Perform all other duties as assigned by Supervisor
Perform all duties in accordance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state and federal regulations
