Requirements

  • Registered Nurse (RN) licensure
  • Ability to perform under the direction of the Supervisor and with guidance from the Educator/Preceptor or in collaboration with another RN
  • Completion of CAP requirements to maintain or advance
  • Physical ability to provide direct patient care, including heavy lifting, moving patients, assisting with ambulation, frequent prolonged standing (reasonable accommodations for disabilities)
  • Based in regional Midwest area (Geraldton) as a spoke nurse

Responsibilities

  • Perform ongoing systematic collection and analysis of dialysis data for assigned patients, document in medical record, adjust/modify treatment plan as indicated, notify Supervisor or physician as needed
  • Assess, collaborate, and document patient/family’s basic learning needs; provide initial and ongoing education to patients and family
  • Direct and provide, in collaboration with patient, home care partner, direct and ancillary staff, all aspects of safe and effective delivery of dialysis therapies to assigned patients
  • Administer medications as prescribed or per approved algorithm(s), document medical justification and effectiveness
  • Initiate or assist with emergency response measures
  • Serve as resource, leader, coach, mentor, and role model for new and incumbent employees by setting examples of appropriate behavior, work habits, and attitudes
  • Ensure correct laboratory collection, processing, and shipping procedures; reschedule missed or insufficient collections
  • Identify expected outcomes, document and update nursing assessment and plan of care through collaboration with Interdisciplinary Team
  • Ensure patient awareness of transplant and treatment modality options
  • Assist in identification, evaluation, selection, and education of Home Dialysis candidates and Home Partners
  • Perform assessment of Home Dialysis candidate’s home environment, partner/family readiness and ability to perform dialysis treatments
  • Train home dialysis patients and/or Home Partners on safe, effective operation and maintenance of Home Dialysis equipment and supplies via formal standardized training program
  • Participate in education and quality improvement projects at facility and area level as directed by Supervisor
  • Serve as Preceptor to new employees
  • Perform all other duties as assigned by Supervisor
  • Perform all duties in accordance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state and federal regulations
