Requirements

  • Registered professional nurse (RN) with entry-level designation into the Clinical Advancement Program (CAP)
  • Ability to perform under the direction of the Supervisor with guidance from the Educator, Preceptor, or in collaboration with another RN
  • Complete CAP requirements to advance
  • Capability to handle physical demands: heavy lifting, moving equipment and patients, assisting with ambulation, frequent prolonged standing, bending, moving machines/equipment up to 200 lbs. (with assistance), lifting chemical/water solutions up to 30 lbs. as high as 5 ft

Responsibilities

  • Perform ongoing, systematic collection and analysis of dialysis data for assigned patients, document in medical record, adjust/modify treatment plan as indicated, and notify Supervisor or physician as needed
  • Assess, collaborate, and document patient/family’s basic learning needs to provide initial and ongoing education
  • Direct and provide, in collaboration with patient, home care partner, direct and ancillary staff, all aspects of safe and effective delivery of dialysis therapy
  • Administer medications as prescribed or per approved algorithm(s), and document medical justification and effectiveness
  • Initiate or assist with emergency response measures
  • Serve as a resource for health care team, participate in staff training and orientation of new staff
  • Ensure correct laboratory collection, processing, shipping procedures, and reschedule missed/insufficient collections
  • Identify expected outcomes, document and update nursing assessment and plan of care through collaboration with Interdisciplinary Team
  • Ensure patient awareness of transplant and treatment modality options
  • Assist in identification, evaluation, selection, and education of Home Dialysis candidates and Home Partners
  • Perform assessment of Home Dialysis candidate’s home environment, partner/family readiness, and ability to perform treatments
  • Train Home Dialysis patients and/or Home Partners on safe, effective operation and maintenance of Home Dialysis equipment and supplies through organized formal training program
  • Participate in decision-making, teaching, leadership functions, and quality improvement activities
  • Perform all other duties as assigned by Supervisor
