Candidates must possess a graduate degree in Nursing from an accredited school of nursing and be a Registered Nurse (R.N.) with a current, unrestricted license to practice in the state of Virginia. A minimum of three years of experience as a Registered Nurse is required, with at least one year of experience in case management preferred. Experience in chronic disease and behavioral health care is also desirable.
The Registered Nurse Case Manager will manage a panel of members, creating and overseeing care plans, frequently reassessing needs and coordinating with providers and resources. They will conduct clinical examinations when necessary, participate in case conferences, develop care plans in collaboration with members, perform medication reconciliation, and address social needs. The RNCM will conduct regular clinical visits, monitor therapeutic interventions, utilize care facilitation tools, document member interactions, and support members in achieving their care plan goals, while also addressing quality gaps and ensuring thorough chart documentation.
