Requirements

  • Registered Dietitian credentials

  • Ability to assess patient's learning needs and provide appropriate education

  • Proficiency in completing progress notes and maintaining documentation per FMCNA policies, Federal, and state regulations
  • Competence in monitoring lab results (e.g., albumin, body weight, nutrition-related parameters) and adjusting therapy
  • Ability to collaborate with interdisciplinary teams on Urea Kinetic Modeling, mineral metabolism, renal bone disease, anemia, iron status, psychosocial status, vascular access, modality, rehabilitation, and sodium/volume control
  • Adherence to FMCNA algorithms and medical record policies
  • Participation in QAI/QEP activities and process improvements

Responsibilities

  • Assess patient’s knowledge of diet and kidney disease; provide tailored education to patient/family/caregiver on nutritional status, adequacy measures, fluid status, and lab parameters
  • Complete nutritional assessments and participate in interdisciplinary comprehensive assessments
  • Develop written, individualized comprehensive plans of care per Federal/state guidelines
  • Provide ongoing nutrition assessment, counseling, and interventions for malnourished/at-risk patients
  • Review patient’s albumin level and body weight monthly; implement and monitor plan of care with patient/family/caregiver involvement
  • Monitor adherence/response to nutrition therapy, adjust as needed, and document barriers to goals
  • Collaborate on interdisciplinary recommendations for Kt/V, mineral metabolism, renal bone disease, anemia, iron, nutritional/psychosocial status, vascular access, modality, rehabilitation, and sodium/volume control
  • Report on nutrition QAI results and participate in QAI program
  • Coordinate Pre-ESRD education and work with Case Manager for patient education and supplement referrals
  • Review nutrition-related lab results, counsel patient/caregiver, recommend treatment changes, and communicate with physicians/staff/providers in facilities (e.g., hospitals, nursing homes)
  • Participate in pertinent staff meetings (e.g., general staff, quality improvement)
