Requirements

  • Licensed Practical Nurse (LPN) - active license in the state of practice (Upon Hire)
  • 3-5 years nursing experience in long term care
  • Visual acuity adequate to perform position duties
  • Ability to communicate effectively with others, hear, understand, and distinguish speech and other sounds
  • Ability to work primarily days with possible weekends/evenings/holidays

Responsibilities

  • Review and coordinate all referrals to the facility
  • Perform pre-admission screens, pre-admission rounds, and coordinate all new admissions
  • Assist in the development of the baseline care plan and attend weekly care plan meetings as needed
  • Communicate and manage the delivery of resident care, ensuring quality care is provided
  • Manage provider visits (30-60-90 days) and routine lab orders
  • Accurately enter resident admission/registration information, assist with completion of required forms, prepare charts, enter orders, schedule appointments, and obtain/distribute resident education materials as directed
  • Complete assessments on residents, make appropriate interventions, apply knowledge, prioritize resident care, and use the nursing process (assess, plan, implement, evaluate)
  • Assist IC/Quality with quality improvement projects, data collection, and chart audits to achieve desired quality outcomes
  • Provide information/training for staff as needed
  • Maintain competence in nursing capabilities outlined by leadership
  • Assist with the investigative process for thorough investigations for facility and state mandated concerns
  • Coordinate admissions, discharges, and transfers to and from the facility
  • Serve as a core member of the interdisciplinary care team with authority to assess, plan, evaluate, and teach in patient and staff situations
  • Carry out administrative responsibilities as delegated by facility leadership
