Responsibilities

  • Attend patients at assigned locations, including multiple locations, levels of care, and virtual environments
  • Conduct medication management sessions
  • Perform psychiatric evaluations and assessments
  • Create patient treatment plans and manage updates

  • Participate in team and unit meetings
  • Create discharge plans
  • Collaborate with nurse practitioners and physician assistants
