- Licensed in Utah with Psychiatric Mental Health Nurse Practitioner (PMHNP) Certification (do not accept FNPs)
- Must have or be able to obtain Prescriptive Authority
- Must have or be able to obtain DEA number
- 2 years experience as a PMHNP
- Experienced with adult and/or child and adolescent populations
- Experienced in both medication management and therapy

The salary range is $169,000 - $254,000 a year for W2 full-time employment.

This is a hybrid role offering telemedicine and in-person flexibility for all Utah locations in the Greater Salt Lake City area, including Bountiful, Riverton, Lehi, Murray, Pleasant Grove, and Draper.
