Requirements

Active Physical Therapist license in the state of practice (Upon Hire)
Visual acuity adequate to perform position duties
Ability to communicate effectively with others, hear, understand, and distinguish speech and other sounds

Responsibilities

Evaluate assigned patients, gathering data from patient records, interviews, observation, and team members
Develop and implement individualized treatment programs for patients
Use selected activities/tasks to promote health, achieve treatment goals, and modify interventions based on ongoing evaluation
Provide direct therapy services using functional settings and strategies, coordinated with rehabilitation, medical, and nursing care plans
Prepare documentation including evaluation results, individualized treatment plans, progress notes, and reports
Communicate effectively with patients, families, physicians, and interdisciplinary team members regarding status, progress, and needs
Establish rapport and work cooperatively with interdisciplinary team members (including family/physician)
Enhance professional growth through educational programs, literature, in-service meetings, and workshops
Coordinate and deliver daily physical therapy services according to physical therapy principles and department objectives
