Entry level designation into the Clinical Advancement Program (CAP) for Outpatient RN CAP 1
Ability to perform direct patient care involving heavy lifting, moving patients, assisting with ambulation, frequent prolonged standing, bending over, and lifting equipment/chemicals/water solutions up to 30 lbs. (with assistance for heavier items up to 200 lbs.)
Comfortable in a health care facility environment with moderate noise, potential exposure to infectious/contagious diseases, air temperature control, desk/computer work, and patient/staff interactions
Willingness to travel to training sites or other facilities and perform essential functions in various locations
Completion of CAP requirements to advance

Responsibilities

Perform all essential functions under the direction of the Supervisor and with guidance from the Clinical Educator, Preceptor, or in collaboration with other Registered Nurses
Conduct ongoing, systematic collection and analysis of patient data pre-during-post hemodialysis treatment, document in medical record, adjust treatment plans, and notify Team Leader, Charge Nurse, Supervisor, or Physician as needed
Assess, collaborate, and document patient/family basic learning needs; provide initial and ongoing education
Direct and provide, in collaboration with direct and ancillary staff, all aspects of daily safe and effective hemodialysis therapy delivery to assigned patients
Administer medications as prescribed or per approved algorithms, documenting justification and effectiveness
Initiate or assist with emergency response measures
Serve as a resource for the health care team, participate in staff training and orientation of new staff
Ensure correct laboratory collection, processing, shipping; reschedule missed or insufficient collections
Identify expected outcomes, document and update nursing assessment and plan of care through collaboration with the Interdisciplinary Team
Ensure patient awareness of transplant and treatment modality options
Complete CAP requirements to advance
Perform all other duties as assigned by Supervisor
