Requirements

Minimum 1 year of active field experience as a New Jersey-certified EMT required
Experience in home healthcare or mobile integrated healthcare is highly preferred
High school diploma or GED
Graduate of a recognized EMT Training Program
Current New Jersey EMT certification issued by the NJ Department of Health
Current BLS CPR certification (American Heart Association or equivalent)
Valid driver’s license recognized by the State of New Jersey; driving record must meet insurance company standards
Must complete annual clinical competencies, Fit Test, PPD screening, AMOC training, and any other required training by designated deadlines

Responsibilities

Conducts in-home patient assessments following discharge to evaluate condition, medication adherence, and safety risks. Reports findings to supervising nurse or provider as appropriate
Completes accurate clinical documentation in the EMR/ePCR. Responds to quality assurance and medical oversight requests
Facilitates telehealth visits with providers and communicates findings to the interdisciplinary care team
Operate and monitor designated electronic equipment (e.g., vitals monitors, tablets, telehealth tools) per training and within EMT scope
Communicates professionally with patients, families, and multidisciplinary team members to coordinate care and escalate concerns
