Requirements
- Entry-level MSW position requiring a Master's in Social Work (MSW)
- Clinical training and state license permitting supportive counseling services
- Knowledge of social work theory, evidence-based practices, and FKC patient education programs
- Familiarity with quality of life measurement instruments and FMCNA Social Work Policy, including Measuring Patient Physical and Mental Function Policy
- Current knowledge of local vocational/educational rehabilitation programs, community resources (e.g., home health services), and advance directives
- Ability to collaborate with interdisciplinary teams (nurses, physicians, etc.) in dialysis settings (in-center and home)
Responsibilities
- Assess patients' psychosocial status, strengths, and needs as part of comprehensive patient assessment and care planning
- Participate in interdisciplinary care planning to identify interventions for rehabilitation, treatment goals, and quality of life improvement
- Provide monitoring, interventions, supportive counseling, and education to help patients adjust to dialysis and achieve optimal psychosocial status
- Offer goal-directed counseling for transplant-seeking patients and facilitate referrals to community resources, vocational rehabilitation, etc
- Assist with advance directives, DNR status discussions, and patient/family education on rights, responsibilities, and grievances
- Provide psychosocial support and Social Work Focused Interventions for non-adherence, quality outcomes, and quality of life concerns
- Participate in monthly Quality review meetings, reporting on indicators like Missed/Shortened Treatments, Quality of Life Trends, and Service Recovery
- Assess patient knowledge of kidney disease, provide tailored education on treatment modalities, ESRD psychosocial issues, and support services
- Facilitate transplant referral process and waitlist management in collaboration with the team
- Document all MSW interactions, interventions, and care provided to patients and families
