Responsibilities

Assess patients' psychosocial status, strengths, and areas of need as part of the comprehensive patient assessment
Participate in care planning with the patient and interdisciplinary team to identify effective interventions for rehabilitation, treatment goals, and improved quality of life
Utilize FKC patient education programs, social work theory, methods, interventions, and quality of life measurement instruments in assessment and care planning
Provide monitoring and interventions to help patients adjust to dialysis and achieve optimal psychosocial status and quality of life
Provide supportive counseling services to patients within the scope of clinical training and state license
Provide educational and goal-directed counseling to patients seeking transplant
Provide information and assist with referrals to community resources (e.g., home health services, vocational rehabilitation) for optimal treatment outcomes
Maintain current knowledge of local vocational/educational rehabilitation programs and assist patients with referrals and access to remain employed, become employed, or receive education
Assess patient awareness of advance directives, assist with accessing forms/information, and facilitate discussions with the healthcare team and family/support persons
Participate in discussions of patient DNR status with physician, nurse, patient, and family to ensure informed decisions
Adhere to FMCNA Social Work Policy and Measuring Patient Physical and Mental Function Policy, including documentation of interactions and interventions
Provide psychosocial support and/or Social Work Focused Interventions to address non-adherence, quality outcomes, and quality of life concerns based on acuity level
Participate in monthly Quality review meetings, reporting on quality indicators like adherence (Missed/Shortened Treatments), Quality of Life Trends, and Service Recovery
Assess patient knowledge of kidney disease for barriers affecting treatment adherence and provide tailored education with patient, family, and healthcare team
