Responsibilities

  • Assess patients' psychosocial status, strengths, and areas of need as part of the comprehensive patient assessment
  • Participate in care planning with the patient and interdisciplinary team to identify effective interventions for rehabilitation, treatment goals, and improved quality of life
  • Utilize FKC patient education programs, social work theory, methods, interventions, and quality of life measurement instruments in assessment and care planning
  • Provide monitoring and interventions to help patients adjust to dialysis and achieve optimal psychosocial status and quality of life
  • Provide supportive counseling services to patients within the scope of clinical training and state license
  • Provide educational and goal-directed counseling to patients seeking transplant
  • Provide information and assist with referrals to community resources (e.g., home health services, vocational rehabilitation) for optimal treatment outcomes
  • Maintain current knowledge of local vocational/educational rehabilitation programs and assist patients with referrals and access to remain employed, become employed, or receive education
  • Assess patient awareness of advance directives, assist with accessing forms/information, and facilitate discussions with the healthcare team and family/support persons
  • Participate in discussions of patient DNR status with physician, nurse, patient, and family to ensure informed decisions
  • Adhere to FMCNA Social Work Policy and Measuring Patient Physical and Mental Function Policy, including documentation of interactions and interventions
  • Provide psychosocial support and/or Social Work Focused Interventions to address non-adherence, quality outcomes, and quality of life concerns based on acuity level
  • Participate in monthly Quality review meetings, reporting on quality indicators like adherence (Missed/Shortened Treatments), Quality of Life Trends, and Service Recovery
  • Assess patient knowledge of kidney disease for barriers affecting treatment adherence and provide tailored education with patient, family, and healthcare team
