Requirements

  • Master's degree in Social Work (MSW)
  • Licensed as a Master Social Worker (MSW 3 level)
  • Knowledge of Social Work Theory of Human Behavior and accepted methods of social work practice
  • Ability to assess psychosocial status, strengths, and areas of need affecting rehabilitation and treatment outcomes
  • Skills in supportive counseling, educational and goal-directed counseling within scope of clinical training and state license
  • Current knowledge of local vocational/educational rehabilitation programs and community resources
  • Experience or capability in supervising social work supervisees, including monitoring assessments, diagnoses, treatment decisions, clinical dynamics, compliance with laws, progress notes, and direct observation/review of sessions
  • Ability to collaborate with interdisciplinary healthcare teams, including physicians and nurses

Responsibilities

  • Provide psychosocial services to in-center and home dialysis patients using social work theory and methods
  • Work with the healthcare team to promote positive adjustment, rehabilitation, and improved quality of life
  • Inform, educate, and support staff on the emotional, psychological, and behavioral impact of Chronic Kidney Disease on patients and families
  • Support FMCNA Quality Indicators, Outcomes, QAI activities, patient satisfaction, quality of life, and process improvement to achieve quality goals
  • Supervise supervisees by ensuring appropriate counseling quality, monitoring assessments/diagnoses/treatments, providing feedback, evaluating service provision, addressing clinical dynamics, ensuring legal compliance, reviewing records, and observing/reviewing sessions with consent
  • Assess patients' psychosocial status as part of the interdisciplinary comprehensive patient assessment
  • Participate in care planning with patients and team to identify interventions for rehabilitation, treatment goals, and quality of life
  • Utilize FKC patient education programs, social work interventions, and quality of life tools to address barriers and meet goals
  • Monitor and intervene to help patients adjust to dialysis and achieve optimal psychosocial status
  • Provide supportive counseling to patients within scope of training and license
  • Offer educational and goal-directed counseling for transplant-seeking patients
  • Assist with referrals to community resources (e.g., home health, vocational rehabilitation)
  • Help patients access vocational rehabilitation to maintain, obtain, or pursue employment/education
  • Assess patient awareness of advance directives, assist with forms/information, and facilitate discussions with team and family/support persons
  • Collaborate with physician and nurse on patient DNR discussions
