Requirements
- Adheres to FMCNA Compliance Program, regulatory requirements, and FMS policy and procedure requirements

- Knowledgeable of and adheres to FMCNA Social Work Policy, including documentation
- Entry-level Master Social Worker (MSW) role
- Utilizes Social Work Theory of Human Behavior and accepted methods of social work practice
- Utilizes FMS Ultra Care patient education programs, established social work theory and methods, and quality of life measurement instruments
- Knowledgeable about advance directives, Do Not Resuscitate Orders, and related discussions
Responsibilities
- Provides psychosocial services to in-center and home dialysis patients (if applicable)
- Works with the health care team to promote positive adjustment, rehabilitation, and improved quality of life for patients
- Informs, educates, and supports staff in understanding the emotional, psychological, and behavioral impact of Chronic Kidney Disease on patients and families
- Supports FMCNA commitment to Quality Indicators, Outcomes, Quality Assessment and Improvement (QAI) Activities, patient satisfaction, and quality of life; participates in process improvement activities
- Drives FMS culture through values and customer service standards; accountable for outstanding customer service to external and internal customers
- Develops and maintains effective relationships through effective and timely communication
- Takes initiative to respond, resolve, and follow up on customer service issues timely
- Assesses patients' psychosocial status, strengths, and areas of need as part of comprehensive patient assessment
- Participates in care planning with patient and healthcare team to identify interventions for rehabilitation goals and quality of life
- Provides outcomes-driven psycho-educational counseling to improve treatment adherence, vocational/educational rehabilitation, and/or quality of life
- Provides supportive and goal-directed counseling to transplant-seeking patients
- Assesses patient awareness of advance directives; assists with accessing forms/information and facilitates discussions
- Provides general information about Do Not Resuscitate Orders and Advanced Directives
- Provides information and assists with referrals to community resources (e.g., home health, vocational rehabilitation)
- Participates in discussions of patient DNR status with physician, nurse, patient, and family to ensure understanding and informed decision-making
- Provides usual care and/or Social Work Intensive for non-adherence and quality of life concerns
- Assesses patient knowledge of kidney disease and barriers to treatment adherence
- Works with patient, family, and health care team to provide tailored education based on patient's learning style and needs
