Requirements
- Adheres to FMCNA Compliance Program, regulatory requirements, and FMS policy and procedure requirements

- Knowledge of Social Work Theory of Human Behavior and accepted methods of social work practice
- Ability to work with interdisciplinary health care team

Responsibilities
- Provides psychosocial services to in-center and home dialysis patients
- Assesses patients' psychosocial status, strengths, and areas of need as part of comprehensive patient assessment
- Participates in care planning with patient and healthcare team to identify interventions for rehabilitation goals and quality of life improvement
- Provides outcomes-driven psycho-educational counseling to improve treatment adherence, vocational/educational rehabilitation, and quality of life
- Offers supportive and goal-directed counseling to transplant-seeking patients
- Assesses patient awareness of advance directives, assists with forms/information, and facilitates discussions
- Provides general information about Do Not Resuscitate Orders and Advanced Directives
- Refers patients and team to community resources (e.g., home health, vocational rehabilitation)
- Participates in discussions of patient DNR status with physician, nurse, patient, and family
- Provides usual care and/or Social Work Intensive for non-adherence and quality of life concerns
- Assesses patient knowledge of kidney disease and barriers to treatment adherence; provides tailored education with patient, family, and team
- Drives FMS culture through values and customer service standards; accountable for outstanding service to all customers
- Develops and maintains effective relationships through timely communication
- Responds, resolves, and follows up on customer service issues
- Informs, educates, and supports staff on emotional, psychological, and behavioral impact of Chronic Kidney Disease
- Supports Quality Indicators, Outcomes, QAI activities, patient satisfaction, and process improvement for FMCNA Quality Goals
