RN Field Care Manager

Candidates must be a graduate of an accredited school of nursing (RN), possess Basic Life Support (BLS) certification, and have at least 3 years of experience. An active RN License in the state(s) of practice and an unrestricted Driver's License are also required. The role is hybrid, with 80% virtual and 20% community-based work, and requires residency in the Dayton, OH area.
The RN Care Manager will coordinate with hospital case managers to assess member needs, conduct discharge visits (in-home or virtual), and perform clinical exams. Responsibilities include completing screenings, participating in inpatient rounds and post-discharge conferences, and developing post-discharge care plans in collaboration with the care team. The role involves regular check-ins, community-based support, medication reconciliation, quality gap closure, and utilizing electronic health records for data collection and documentation. The RNCM will also track TOC-related metrics and new TOC events.
