Requirements
- Minimum five (2) years’ experience in Hospice setting
- Minimum three (2) year experience using HCHB to perform operations relating to the Hospice Business model
- Verbal and written communication skills necessary to explain complex and/or confidential information and communicate with all levels of management
- Ability to anticipate and identify problems and use sound judgment and fact-based analysis to develop effective and efficient solutions
Responsibilities
- Train and precept new intake coordinators as needed
- Initiation and coordination of the referral process
- Works directly with all team members and management to improve processes and implement
- Learns and manages each branches’ individual requirements, time standards for processing, and analytics to meet and exceed excellent patient care
- Assists with managing daily workflow and intake coverage
- Other duties as assigned
- Working closely with the marketing and clinical staff by coordination of the total referral process including the referral initiation, insurance verification, assignment of referrals and record keeping
- Establishing and maintaining positive relationships with customers and referral sources
