Requirements

  • Minimum five (2) years’ experience in Hospice setting
  • Minimum three (2) year experience using HCHB to perform operations relating to the Hospice Business model
  • Verbal and written communication skills necessary to explain complex and/or confidential information and communicate with all levels of management
  • Ability to anticipate and identify problems and use sound judgment and fact-based analysis to develop effective and efficient solutions

Responsibilities

  • Train and precept new intake coordinators as needed
  • Initiation and coordination of the referral process
  • Works directly with all team members and management to improve processes and implement
  • Learns and manages each branches’ individual requirements, time standards for processing, and analytics to meet and exceed excellent patient care
  • Assists with managing daily workflow and intake coverage
  • Other duties as assigned
  • Working closely with the marketing and clinical staff by coordination of the total referral process including the referral initiation, insurance verification, assignment of referrals and record keeping
  • Establishing and maintaining positive relationships with customers and referral sources
